| All applications must be processed online using the Application Form. No faxed , paper, or email copies of the form will be accepted. If you have questions about the form or are unable to submit the form successfully, please contact the registrar . Applicants will receive a confirmation email within one day of the application submission. Note that the application is NOT COMPLETE until payment is received at the Chicago Magic Soccer Club office (address on Application). |
| After submitting your on-line application, please check the "Team Lists" on the "Schedules/Results" page to verify that your application is in our database. This may take up to two days to process. Any problems with the application, please contact the webmaster and include your team name and age group. |
