| Boys Fall 2009 Best of the Midwest Tournament |
| This tournament has traditionally been one of the premier youth tournaments in the USA . Annually about 300 teams participate with teams coming from throughout the Midwest and USYSA Region II, as well as occasional teams from across the country. There are Boys divisions with multiple flights at each age group. This tournament is hosted at the Hallmark Convention and Sports Complex in Hebron , IN on some of the premier fields in the Midwest . |
| The Fall 2009 Boys Best of the Midwest Tournament is open to clubs registered with USYSA , US Club Soccer, and Super Y-League. Teams affiliated solely with US Club Soccer or Super Y-League must meet the following criteria: |
- Must declare a maximum of 12 players (U9-U10); 14 players (U11-U12); 18 players (U13-U16); 22 players (U17-U19) roster for tournament (identify on tournament roster form)
- Proof of insurance Must be shown for each player (copy of insurance cards)
- Indiana Medical Release forms MUST be filled out
- If traveling with US CLUB cards - you do not need a travel permit

| Dates |
- September 12-13, 2009. If needed, only local teams will be scheduled to play on Friday, September
11, 2009.
- Application fee must be received in the Magic Office by August 21, 2009 to be considered.
- Acceptance notification on the web August 24, 2009.
- Book hotel reservations conveniently and securely through the Lake County CVB Housing.
- Bracket information available on the web August 28, 2009.
- Schedules available on the web as early as September 1 and no later than September 5, 2009.

| Requirements for ALL Teams Participating |
- Apply on-line. (No faxed, paper, or email copies will be accepted.) If you have problems with the on-line application form, please contact the webmaster.
- Submit payment prior to the application deadline.
- Make all special requests on the application form (e.g. coaching conflicts, etc.)
- Acceptance will be based only on the information submitted on your application and is subject to verification.
- No refunds once your team has been accepted into the tournament.
- Each team manager, or coach, is expected to have read and completed the " Manager Check List ".
- Each team manager and coach should read the tournament rules and be familiar with them.
- Register in person on Friday, September 11, 2009 at 1650 East 181st Avenue , in Hebron , IN 46341 from 4:00-9:00 PM.
- To avoid lines and Friday drive you can pre-register.
