Registration Requirements
Please provide this information once you are accepted to participate on a Chicago Magic PSG Team.
Birth Certificate or Passport
Copy of player's birth certificate or passport is required to register.
CMSC requires this of all players for league submission.
Options for Submission:
1. Upload to your gotsoccer.com account under the 'DOCUMENTS' tab.
2. Email to your team manager.
Photo
All players are required to provide a photo for a league pass. Photos need to be appropriate, head & shoulders shot. No action shots, sunglasses, hats or please.
Instructions on Photo Submission:
1. Upload to your gotsoccer.com account
2. Email picture to your team manager.
Include in the body of the email
· Player Name
· Player Age
· Player Birth Date
