• Payment must be received by due date (You may pay by credit card or check)

• Download and print from tournament web site
• Make copy for coaches and players to review

• Current/valid US Club Player Passes required for all players, OR
• Current/valid USYS Player Passes required for all players

• USYSA form, approved by Your State Association
• No travel form required for US Club Soccer or Super Y-League teams if using US Club cards

• State Association determined, approved by State Association for USYSA teams
• US Club or USYS Official Roster
• Team Representative should keep original copies throughout tournament

• US Club or USYS Medical Releases
• Team Representative should keep original copies throughout tournament

• Completed and signed by coaches of BOTH teams (sending and receiving)
• Illinois FORM for Illinois teams located on tournament web site - Out of state teams use their state form
• US Club/Super Y-League players must be on your official US Club roster

( Registration will be held on the Friday at 1650 East 181st Avenue , in Hebron , IN 46341 .  (Registrations will be taken as early as 1 ½ hours before the start of the first tournament game and as late as 1 ½ hours before the start of your first game. ) Each team must check in to let us know that you have arrived and to provide us a copy of your official roster and emergency contact information. Please also have with you at all times:

• State Association approved “Application to Travel” (non-Indiana teams only) for USYSA teams. US Club teams do not need a travel form.
• Current passes for all players
• Copies of USYS Medical Release forms or US Club Medical Forms for ALL players
