| The Spring 2007 Kick Off Soccer Tournament is open to clubs registered with USYSA, US Club Soccer, and Super Y-League. |

| Dates |
- Application deadline is March 1, 2007.
- Application fee must be received in the Magic Office by March 1, 2007 to be considered.
- Acceptance notification on the web March 3, 2007.
- Book hotel reservations conveniently and securely through Lake County CVB Housing.
- Bracket information available on the web March 10, 2007.
- Schedules available on the web March 24, 2007.
- Registration will be held on the Friday at 1650 East 181st Avenue, in Hebron, IN 46341. (Registrations will be taken as early as 1 ½ hours before the start of the first tournament game and as late as 1 ½ hours before the start of your first game. )

| Registration Requirements for ALL Teams Participating |
- Apply on-line - All applications must be processed online using the Application Form. (No faxed, paper, or email copies will be accepted.) If you have problems with the on-line application form, please contact the registrar.
- Submit payment prior to the application deadline.
- Make all special requests on the application form (e.g. coaching conflicts, etc.)
- Acceptance will be based only on the information submitted on your application and is subject to verification.
- No refunds once your team has been accepted into the tournament.
- Each team manager, or coach, is expected to have read and completed the "Manager Check List".
- Each team manager and coach should read the tournament rules and be familiar with them.

| Requirements for ALL Teams Participating |
| 1. Registration will be held on the Friday at 1650 East 181st Avenue, in Hebron, IN 46341. (Registrations will be taken as early as 1 ½ hours before the start of the first tournament game and as late as 1 ½ hours before the start of your first game.) Each team must check in to let us know that you have arrived and to provide us a copy of your official roster and emergency contact information. Please also have with you at all times: |
- a copy of your US Club Soccer or USYS official roster
- a US Club Soccer or USYS player pass for each player
- a US Club Soccer or USYS medical release form for each player

| Note: The Chicago Magic Spring Kick-Off Invitational tournament is a US Club Soccer sanctioned event. For teams that are not members of US Club Soccer: |
| 2. Please bring a completed permission to travel form if required by your state association. |
| 3. Please be aware that your state association may not provide insurance coverage for your participation in the tournament. |
| 4. Your state association can tell you their policy. For teams who are not covered, US Club Soccer provides an option for you to purchase tournament coverage. The cost is $50/team and may be applied for through an online form. For more information, please visit the US Club Soccer web site here. |
